A wheel-off accident in Lockland, Ohio, killed the driver of a van that was traveling southbound on Interstate 75.
The rear tire/wheel assembly came off a northbound tractor-trailer and bounced across the median, where it struck the van.
According to local reports, authorities have located the truck's driver, who may not have known the tire came loose.
The right two southbound lanes were shut down for several hours after the incident.
